CareFlow
A smart healthcare experience designed to help people manage diabetes and hypertension through daily tracking, nutrition guidance and medication support.
Making chronic condition management less exhausting
CareFlow is a mobile app concept built for people managing diabetes and hypertension day to day. The goal was to reduce the mental load of tracking readings, medication and diet into a single, calm daily flow.
Existing tools felt clinical, not personal
Most condition-tracking apps on the market are built like spreadsheets — dense, clinical, and easy to abandon after the first week. Users needed something that felt more like a daily companion than a logbook, without hiding the data that actually matters to their care team.
One home screen, three daily actions
The design consolidates the three habits that matter most — logging readings, checking medication, and reviewing meals — into a single home screen with clear, large touch targets and gentle, encouraging language instead of clinical warnings.
From flows to a working prototype
Started with low-fidelity user flows mapping the three core habits, then moved into wireframes to test information hierarchy, and finally into high-fidelity screens with a calming, accessible color system tested for readability by older users.
A calmer way to stay on track
The final UI leans on soft gradients, generous spacing and a restrained color palette so the app never feels alarming — even when it's surfacing important health data.
A concept ready for user testing
CareFlow exists today as a fully designed, prototyped concept — the next step is structured usability testing with the exact audience it was built for.
